The Manila City Government has officially reported a leptospirosis outbreak after infections increased, emphasizing the increasing health risks associated by recent flooding in the capital.
Health regulatory bodies reported a rapid increase in leptospirosis cases after heavy rains and broader flooding. Officials are promptly implementing emergency protocols to control the outbreak and mitigate the risk of transmission.
The Department of Health noted that the case of leptospirosis in the city increased from 1.27% to 3.48%, exceeding the required amount for an epidemic level. This increase demonstrates the rising number of residents exposed to floodwaters in the recent rainy season and has increased concerns among health regulatory bodies.
Cases Surge
Health regulatory bodies have reported 1,000 leptospirosis cases in Manila this 2026. Reports show that roughly 982 infections have been reported, while 35 deaths have been linked with leptospirosis. The figures demonstrate an increase compared to the previous reported cases and show the intensity of the leptospirosis epidemic.
The increase in leptospirosis has put pressure on the healthcare system. Hospitals throughout the city have experienced an increase in patient admissions due to leptospirosis. Most of the government hospitals have reported reaching full capacity because of the sheer number of cases. City government officials project that more cases may come due to current weather in the capital.
What is Leptospirosis?
Leptospirosis is caused by bacteria. The disease is usually transmitted through water, or mud contaminated with the urine of infected animals such as rats. During floods, contaminated water spread to communities, causing the rise of risk of human exposure. People can be infected when bacteria enter their body through wounds in the skin; and bacteria may also enter through the eyes, nose, or mouth after contact with contaminated water.
Symptoms like fever, headache, muscle pains, chills, vomiting, and fatigue are common symptoms of leptospirosis. In more dire cases—or if left untreated, it can also lead to damage to the kidney, liver, and respiratory systems. Meningitis is another complication; and some cases may lead to sudden death without urgent medical care. Prompt action is essential to reduce the risk of serious complications. And this highlights the stress put on the system by the ever-prevalent occurrences of flash floods and knee-high water.
Urban Flood Risks
The rainy season causes conditions that enable the disease to spread rapidly, particularly in populated urban areas where floodwaters may be contaminated by waste and infected animals.
Some residents have no choice but to wade through contaminated flood water while commuting, evacuating or securing their home. They are the most vulnerable. Although health officials warn against this, necessity like work, school, or daily activities trump risk among individuals. This reflects a systematic problem—one that officials on all levels must recognize, and proactively act on.
City Government Response
The Manila Mayor has directed local health personnel and barangay officials to reinforce disease prevention measures. The city government has distributed free doxycycline capsules across health centers and district hospitals to help reduce infection risks among residents who are exposed to contaminated floodwaters.
City government officials are also implementing public awareness campaigns to continue educating residents about the symptoms of leptospirosis, required proper hygiene, and the essence of seeking immediate medical attention if symptoms develop after exposure to contaminated water.
The leptospirosis epidemic declaration highlights the public health barrier linked to bad weather conditions and urban flooding. As the city is still experiencing seasonal rains, health regulatory bodies continue focusing on preventing infections and securing vulnerable communities from the spread of leptospirosis.
Better Infrastructure
A preventive measure must be taken in combatting this epidemic. The current response of city officials is reactive: they administer medicine, warn against danger, and direct people to hospitals. Yet, there are then observed shortages in medical supplies, hospital beds, and the continuous presence of flooding whenever rain comes.
Rain can—in a poetic sense—be a cause for good or bad. For farmers, this comes as a relief for the sustenance of crops. Cracks in the system has led to poor governance of infrastructure and protocols. It is the mindset observed by the response of the officials that the government is reactive, not proactive. As the cases are primarily caused by contaminated water, should not the policy shift towards preventing the cases of floods itself before it escalates into a fact-of-life for the Filipino? Recent projects such as flood projects have performed poorly, as observed in the rising cases of floods, and the recent uptick in leptospirosis cases.
The focus then should then be directed towards calling out the governance itself; its means of thinking; its implementation; and ultimately, its care for the Filipino.
